I would have included Nintendo in the list, but they don't exactly do "achievements".

EDIT: Although if you want to make your own Nintendo achievements (like the Smash Bros challenges for example), you are free to do so.

Anyways, you can list the game, the title of the achievement/trophy, and the description of it. You can also list how much gamerscore you got from it, or if the trophy was bronze, platinum, etc.
