I believe that it is meant to be the size of the ad that you want to display on ACC.

Basically, yes. Ads are small images that I assume will be displayed on ACC linking to your site.

Does ACC receive revenue from Ads just from displaying them, or only if they are clicked by its members? Because if it is the latter, I'll be clicking on them like a madwoman in support for ACC.

It's a little of both. There are CPC ads (cost per click), and CPM (cost per mille, or cost per one thousand views). Most of what's on Adsense is CPC.
But be aware that Google is able to filter out people they think are "abusing" the system. While I certainly appreciate your support, the best is just to treat them as you normally would, and just click them if you have genuine interest in what the ad is advertising.

1. When we do get it, where will they be displayed? 2. Average costs? Where can I possibly find them? 3. What is the average visits and clicks for each ad on ACC per day?

Correct. I use Google DFP to manage the ads, though. It determines when to show directly sold ads vs. AdSense ads.

No, they will be paid through PayPal.
Would ACC accept an ad from another Animal Crossing fan site?
Possibly. I touched on this above, but basically, it depends on what the site does. If it does something that we currently don't do, and don't plan on ever doing, then it wouldn't be considered a "competitor". But if it does do similar things, it may be considered a competitor, and thus the request may get rejected.
Who mainly checks the advertisment requests? Is it Jader or can it be any other staff member?

What other way would you/ they like to pay? I assume that those two options were chose because they are the most convenient for a majority of people. Some people would rather pay every 1000 clicks, giving them more time to gather the money needed to pay, other would rather pay every time somebody clicks the ad.
In general, if you are not earning an income from your site, advertising it on ACC may not be an option for you. This is really meant for people who have online businesses that want to advertise with ACC.
However, if you somehow have the money to afford advertising ($50/month minimum), then you're more than welcome to send a request.
EDIT: But to answer your question, yes, both of those are required. If you don't know which pricing model you want, and don't have a monthly budget, you probably shouldn't send a request anyway.

Oh, another thing. The forum I'm talking about allows for words to be said like the A word, the B word, the D word etc etc. We censor the more serious curses like the F word but would stuff like that factor your decision on letting the site be advertised here?

No, you are paying so that your website is being advertised on ACC. Your "reward", so to speak, is the advertisement itself.
